This past week, members of the QI Team at the DentaQuest Institute visited Zufall Health Center in Dover, New Jersey. Zufall has been an active site participating in both DQI’s Early Childhood Caries Collaborative and the Dental Caries Management Collaborative. During the visit, the QI Team toured Zufall’s dental and medical clinics and experienced firsthand their workflow for implementing the chronic disease management protocol for patients between the ages of 6 months to 20 years of age. Zufall is also identifying innovative ways to adapt their electronic dental record (eClinical Works) through the use of templates and quick buttons to track disease management metrics, including caries risk assessment, self-management goals, caries lesion charting, sealants, and treatment plan complete.
Congratulations to Zufall Health Center for all your efforts. You are championing disease management!
